Job Summary
The Registered Nurse (RN) in the Diabetes Center of Excellence plays a vital role in delivering comprehensive, patient-centered diabetes care. This position focuses on education, clinical management, and technical support, ensuring patients receive individualized treatment and ongoing guidance. The RN collaborates closely with endocrinologists, nurse practitioners, and the broader care team to optimize patient outcomes and support long-term disease management.
Key Responsibilities
- Provide expert education on diabetes medications, including administration, side effects, and expected outcomes.
- Prepare and submit prior authorizations for diabetes-related medications.
- Manage and lead appeals for denied medications to ensure timely patient access.
- Collaborate with pharmacies and providers to resolve coverage issues and coordinate appropriate medication alternatives.
- Educate patients on diabetes management, including disease understanding, lifestyle changes, and self-care practices.
- Deliver personalized, patient-centered education tailored to individual needs and learning styles.
- Conduct glucose tolerance tests while ensuring patient safety and accurate results.
- Provide culturally sensitive nutritional counseling, including guidance on dietary approaches such as the Mediterranean diet.
- Offer individualized exercise counseling based on patient health status and limitations.
- Train patients on continuous glucose monitors (CGMs), including setup, usage, troubleshooting, and mobile integration.
- Upload and manage patient device data for clinical review during visits and remote monitoring.
- Educate patients on insulin administration, hypoglycemia and hyperglycemia management, sick day protocols, and glucagon use.
- Provide in-depth training on various CGM brands and insulin pump systems.
- Troubleshoot insulin pump issues and coordinate with vendors for authorizations and support.
- Ensure patients maintain adequate supplies to prevent disruptions in care.
- Respond to patient inquiries related to diabetes management.
- Collaborate with multidisciplinary team members, including dietitians, health coaches, and behavioral health professionals, to address diabetes distress and overall wellness.
- Promote best practices and excellence in diabetes care across the clinical team.
Required Qualifications
- Bachelor of Science in Nursing (BSN).
- Active and unrestricted New York State Registered Nurse (RN) license.
- Certification in Diabetes Education (e.g., CDCES).
- Proven experience with insulin pumps and continuous glucose monitors (CGMs).
- Experience in diabetes education and patient counseling.
- Proficiency with electronic health records (EHR) and medical software.
- Strong communication and interpersonal skills with a focus on patient-centered care.
Preferred Qualifications
- Master’s degree in Nursing or related field.
Schedule:
- Monday, Wednesday, Friday- 8:30am - 5:00pm
- Tuesday & Thursday session is 8:30am - 7:00pm
- One to two Saturdays a month 8:00am - 4:30pm
