We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team at a long-term care facility. The ideal candidate will provide compassionate care to residents, perform nursing duties, and contribute to a positive living environment.
Job Summary
- Perform tasks within the framework of case finding and provision of supportive and restorative care under RN supervision
- Provide nursing care to assigned residents in accordance with facility policies
- Enhance residents' quality of life
- Function as a member of the nursing and interdisciplinary teams
Key Responsibilities
- Provide direct nursing care for residents with complex needs, including hygiene, medical treatment, nutrition, ambulation, and elimination
- Maintain resident safety and comfort
- Direct care provided by Certified Nursing Assistants
- Administer medications to assigned residents (excluding blood products, first IV solutions with medications, IV push medications, and central line medications)
- Perform IV therapy tasks (for certified LPNs), including venipuncture, fluid administration, and dressing changes
- Administer treatments according to facility policies
- Prepare residents for diagnostic tests, procedures, and transportation
- Admit new residents and assist with environmental adjustment
- Participate in Person Centered Care planning and implementation
- Contribute to care plan development and updates
- Assist with health teaching for residents and families
- Establish positive relationships with residents and families
- Participate in unit/team meetings and quality improvement initiatives
- Monitor nutritional needs and assist with mealtime activities
- Assist with post-mortem care as needed
Required Qualifications
- Completion of an accredited LPN program
- Current New York State Practical Nurse license
- BLS certification (American Heart Association) or NY State CPR certification
- Demonstrated competence in nursing skills within the scope of practice
- Ability to establish positive team morale and interpersonal relationships
Preferred Qualifications
- 1+ years of relevant experience in long-term care or similar setting
