Licensed Practical Nurse (LPN) - Nursing Home Day Shift

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team at a long-term care facility. The ideal candidate will provide compassionate care to residents, perform nursing duties, and contribute to a positive living environment.

Job Summary

1. Perform tasks within the framework of case finding and provision of supportive and restorative care under RN supervision
2. Provide nursing care to assigned residents in accordance with facility policies
3. Enhance residents' quality of life
4. Function as a member of the nursing and interdisciplinary teams

Key Responsibilities

1. Provide direct nursing care for residents with complex needs, including hygiene, medical treatment, nutrition, ambulation, and elimination
2. Maintain resident safety and comfort
3. Direct care provided by Certified Nursing Assistants
4. Administer medications to assigned residents (excluding blood products, first IV solutions with medications, IV push medications, and central line medications)
5. Perform IV therapy tasks (for certified LPNs), including venipuncture, fluid administration, and dressing changes
6. Administer treatments according to facility policies
7. Prepare residents for diagnostic tests, procedures, and transportation
8. Admit new residents and assist with environmental adjustment
9. Participate in Person Centered Care planning and implementation
10. Contribute to care plan development and updates
11. Assist with health teaching for residents and families
12. Establish positive relationships with residents and families
13. Participate in unit/team meetings and quality improvement initiatives
14. Monitor nutritional needs and assist with mealtime activities
15. Assist with post-mortem care as needed

Required Qualifications

1. Completion of an accredited LPN program
2. Current New York State Practical Nurse license
3. BLS certification (American Heart Association) or NY State CPR certification
4. Demonstrated competence in nursing skills within the scope of practice
5. Ability to establish positive team morale and interpersonal relationships

Preferred Qualifications

1. 1+ years of relevant experience in long-term care or similar setting
